Ontario Monitoring for Health Program Claim Form

To submit a claim to the Ontario Monitoring for Health Program, you'll need to use a claim form specific to the program. First-time applicants need to have the form signed by a doctor or nurse practitioner confirming insulin use or gestational diabetes. The completed form and original receipts must be mailed to the program's address.

There is no age limit to submit claims for this program year (April 1, 2025 – March 31, 2026) provided that all other eligibility criteria is met.

Eligibility Criteria

Claimants must meet the following eligibility criteria to qualify for assistance through the Monitoring for Health Program:

Permanent resident

  • The claimant must be a permanent resident of Ontario


Valid health card

  • The claimant must have a valid Ontario health card


Insulin using OR have gestational diabetes

  • The claimant must use insulin to manage his/her diabetes (type 1 or type 2) OR have gestational diabetes (diet-controlled or insulin-using).
  • The first claim form submitted to the program by a claimant must be signed by a physician or nurse practitioner to confirm insulin use or gestational diabetes.


No other coverage for supplies

  • Claimants must have no other coverage, in whole or in part, for the supplies being claimed. This applies even if the claimant has reached their annual limit through their insurance plan or other program.


For children and youth 24 years and under, seniors 65 years and older, social assistance recipients and Trillium Drug Program claimants, please see the following:

Children and youth 24 years and younger

Children and youth 24 years and younger receive coverage for their test strips through OHIP+ As such, the Monitoring for Health Program can only reimburse for lancets, blood glucose meters and talking blood glucose meters (subject to annual maximums) for claimants 24 years and younger who use insulin or have gestational diabetes.

Seniors 65 years and over

Seniors 65 years and older receive coverage for their test strips through the Ontario Drug Benefit (ODB) program. As such, the Monitoring for Health Program can only reimburse for lancets, blood glucose meters and talking blood glucose meters (subject to annual maximums) for claimants 65 years and older who use insulin.

Social assistance recipients

Ontario Disability Support Program (ODSP) and Ontario Works (OW) recipients receive coverage for their test strips through the Ontario Drug Benefit (ODB) program. As such, the Monitoring for Health Program can only reimburse for lancets, blood glucose meters and talking blood glucose meters (subject to annual maximums) for social assistance recipients who use insulin or have gestational diabetes.

Trillium Drug Program clients

Trillium Drug Program clients receive coverage for their test strips through the Ontario Drug Benefit (ODB) program. As such, the Monitoring for Health Program can only reimburse for lancets, blood glucose meters and talking blood glucose meters, subject to annual maximums, for Trillium Drug Program clients who use insulin or have gestational diabetes.

1. Can you confirm the status of my claim? I submitted on ____[date]_____. When will I receive my reimbursement?

Due to the high volume of claims received, average cheque processing time is currently 8 weeks from the time your claim is received at our office. We make every attempt to process claims as quickly as possible.

If you are inquiring about claims submitted for the 2024-2025 program year, cheques are currently being mailed out. Depending on Canada Post operations, the cheques should arrive within the next 2-3 weeks.

2. My endocrinologist/physician/registered nurse is still seeing patients virtually. Am I able to submit my claims with an e-signature from my endocrinologist/physician/registered nurse?

Yes, we currently accept e-signatures.

3. I am not able to mail in my claim form. Can I submit it electronically?

Unfortunately, we require the original paper forms and receipts to process your claim. They must be mailed in to 1000-170 University Ave. Toronto, ON M5H 3B3.

4. Can I photocopy my blank claim form and use it to submit more than one claim?

Yes, you can photocopy your blank claim form or print additional forms if you need to submit more than one claim. 

5. How do I access more blank claim forms?

6. I’ve moved since submitting my claim. How do I update my mailing address?

Please send an email to mfhp@diabetes.ca with your name, old mailing address and your new mailing address.

7. I received a cheque, but the amount is incorrect. What should I do?

Please send us an email to mfhp@diabetes.ca with details of your claim.

8. I am on ODSP, am I eligible to submit my claim through this program?

If you are on ODSP, you can only submit for reimbursement of lancets (and a blood glucose meter once every 5 years, if needed). Test trips are covered through the Ontario Drug Benefit (ODB) program.
