Volunteer Application
We need your help to expand our reach and make an even bigger difference in the lives of the millions of Canadians who are living with diabetes.
To get involved, simply complete our online application form and an Association representative will be in touch with you shortly.
The online application will take you approximately 10 minutes to complete. You will need the following information:
• A valid email address
• The names and contact information of two references (preferably from supervisors/professional contacts)
• The name and contact information for your emergency contact
If you do not have an email address, please download our Volunteer Application Form and fax or mail it to your local office.
