Purpose

In recognition of CDA and Professional Section volunteers by five-year increments.

The Award

Recipients of the Professional Member Length of Service Award will be presented with pins and certificates for service in five-year increments which will be presented to them at the Annual Section Meeting at the Canadian Diabetes Association Professional Conference held in October of each year.

Eligibility

  • Be a member of the CDA Professional Section in good standing 
  • Canadian citizen or a landed immigrant
  • Have volunteered professional service related to diabetes care at the national or local level (i.e. for Chapter Executive, National Executive, committee work, CDA Branch, CDA Area and Regional Leadership Centres)
  • Have contributed work on his or her own time, not part of a job description/expectation
  • Have contributed a minimum of 10 hours service in each year. Years need not to be consecutive

Note: Nominators, Members of the Awards Committee and the Director of Professional Development  are not eligible to apply.

  • Applications must be typed or submitted in electronic format (i.e.: word document or pdf.).

Approval process

The decision to grant the award annually is at the discretion of the Awards Committee. The committee reserves the right to not grant the award in any given year if a suitable candidate is not identified. All decisions of the Awards Committee are final and not appealable.

Notification process

The recipient will be notified by the Director of Professional Development in sufficient time so they can arrange to attend the presentation ceremony at the Annual CDA/CSEM Professional Conference
