This award was established in 1994 to honor the memory of Dr. Gerald S. Wong, an advocate of the highest standard in diabetes care and education.
The award is bestowed on a physician in recognition for significant contribution to the diabetes community.
The award consists of a personal plaque for the recipient and reimbursement for travel and accommodation expenses to attend the presentation ceremony at the CDA/CSEM Professional Conference and Annual Meetings in October each year.
A nominee must:
Be a physician;
Have demonstrated long standing and dedicated service to the diabetes community; and,
Have performed service in the form of clinical care, teaching, advocacy, research or a combination of these.
A nomination must:
Be made by two members of the Professional Section of the Canadian Diabetes Association; and,
Include a complete nomination form and a detailed description of the nature and extent of the service and contribution which the candidate has rendered to the diabetes community.
Nominations are reviewed and selected by a Selection Committee comprised of the Professional Section National Executive of the Canadian Diabetes Association.
Deadline for nominations
All nominations must be received by May 6, 2016 and will stand for three years.
