In recognition of a Chapter that has created defined steps to inform and train others of the CPG tools for use at point of care in Diabetes Education Programs and individual practices.
The purpose of the Dissemination & Implementation Award is to recognize a Chapter who demonstrates initiative and innovation for the implementation of CPG’s into practice. Chapters will be expected to include review of the key messages from the CPG’s into their meetings, draft a plan for implementation into practice and share the barriers and successes of chapter members in meeting the objectives of their plan.
The chapter will receive $500.00 and a commemorative plaque presented at the Annual Section meeting as part of the Canadian Diabetes Association Professional Conference in October each year.
The Chapter must be in good standing.
Submissions for the CPG – D&I Award will be reviewed by the Awards Committee in collaboration with members from the Dissemination & Implementation Committee. The Chapter which is chosen to receive this award will receive it at the CDA annual conference and the learning/implementation summary shared with all Chapter members on Diabetes 365.
The Chapter will be notified by the Director of Professional Development in sufficient time so they can arrange to attend the presentation ceremony at the Annual CDA/CSEM Professional Conference.
