In recognition of a camp educators’ commitment and dedication to diabetes camping.

Purpose

The Diabetes Camp Educator Award recognizes a CDA Professional member who has shown commitment and dedication to the Canadian Diabetes Association diabetes program.

The Award

Recipients of the CDA Diabetes CAMP Educator Award will receive $750 and a commemorative plaque presented to them at the Annual Section meeting as part of the Canadian Diabetes Association Professional Conference in October each year.

Eligibility

  • Be a member of the CDA Professional Section in good standing for a minimum of two years
  • Canadian citizen or a landed immigrant
  • Demonstrate volunteer experience in health care promotion currently working in diabetes care.

Note: Members of the Awards Committee and the Director of Professional Development  are not eligible to apply.

Nomination Process

Requirements:

  • Applications must be typed or submitted in electronic format (i.e.: word document or pdf.).
  • One current copy of the applicant’s curriculum vitae
  • Submit a head shot (high resolution jpgs (300 dpi) and 4 x 6 size)

Letters of Recommendation:

  • Applications must include one letter of recommendation from a CDA Professional member in good standing attesting to the specific attributes of the nominee.
  • Applications must include one letter of recommendation from health-care professional or other peers in camping.

Approval Process

The decision to grant the award annually is at the discretion of the Awards Committee. The committee reserves the right to not grant the award in any given year if a suitable candidate is not identified. All decisions of the Awards Committee are final and not appealable.

Notification Process

The Awards Committee will meet to select the successful applicant. All applicants will be notified of their application status.  Unsuccessful candidates are encouraged to re-submit their application for the following year
