An action plan to cut your risk of heart disease
If you have diabetes, you have more than double the likelihood of having a heart attack or stroke. This Action Plan will reduce your risk and help you reverse the trend with the ‘ABCs’ of diabetes management:
A is for strict control of A1C, a measure of blood glucose over time – target: 7 or below.
B is for good blood pressure control – target: 130/80.
C is for cholesterol levels that should be kept within a healthy range – know your target levels and work with your healthcare team to meet them.
- LDL cholesterol target: 2.0 mmol/L or lower
- Total cholesterol/HDL target: Less than 4.0
Ask your doctor which targets are best for you. Cut risk with regular physical activity and a meal plan to maintain a healthy weight – and don’t smoke.

Research shows it makes sense to keep tabs on blood glucose levels and blood pressure, and maintain healthy lifestyle management – even if you don’t have diabetes. You will help reduce your risk of heart disease and potentially prevent or delay the onset of type 2 diabetes.
