Alberta & NWT - Alberta Monitoring For Health (AMFH) Program

Effective July 1, 2012, the Government of Alberta expanded supports available to residents of Alberta with insulin treated diabetes.  Some of these changes impact the Alberta Monitoring for Health Program and some of our clients.

There are now two separate benefits under the Alberta Monitoring for Health Program.

Diabetes Supplies Benefit

(support for low-income Albertans with diabetes)

This benefit may provide coverage to eligible low income Albertans without insurance for basic diabetes supplies (blood glucose test strips, lancets, insulin pen needles, syringes, pump infusion sets and urine test strips) based on the following levels of management:

• Eligible Albertans under age of 65 who manage their diabetes with insulin will have coverage for diabetes supplies up to a maximum of $600 per benefit year

• Eligible Albertans who manage their diabetes with oral medications will have coverage for diabetes supplies up to a maximum of $250 per benefit year

• Eligible Albertans who manage their diabetes through diet and exercise will have coverage for diabetes supplies up to a maximum of $100 per benefit year

Having insurance coverage for insulin or oral medication will not affect eligibility.

PLEASE NOTE: Effective July 1, 2012, all insulin treated seniors (age 65 and older) in Alberta will receive coverage for basic diabetes supplies up to $600 per year (blood glucose test strips, lancets, insulin pen needles, syringes, and urine test strips) under Alberta Blue Cross Coverage for Seniors (1-800-661-6995).

People wishing to access support under the Diabetes Supplies Benefit must meet the following eligibility requirements:
• Have diabetes mellitus.
• Are a full-time resident of Alberta.
• Have valid Alberta Health Care insurance registration.
• Meet one of the following three family status and income levels

Single with an annual, taxable income less than $23,598.
Married/Common law with a combined, spousal, annual, taxable income less than $37,021.
• Single/Married/Common Law (with dependant children) with a combined family, annual, taxable income less than $44,974.

• Manage their diabetes through diet, oral medications and/or insulin therapy.
• Have supplies and/or medication prescribed by a physician
• Do not have insurance coverage for any diabetes supplies, i.e. blood glucose test strips, injection supplies, lancets and pump infusion sets (having insurance coverage for insulin or oral medication will not affect eligibility)
• Have received training in self-monitoring of blood glucose

New Gestational Diabetes Supplies Benefit

(insulin treated only)

This benefit may provide eligible Alberta women with insulin treated Gestational Diabetes, who have no coverage for diabetes supplies, up to $600 per pregnancy for basic diabetes supplies. Supplies are defined as: blood glucose test strips, lancets, insulin pen needles, and syringes. This is not an income-tested benefit and having insurance coverage for insulin or oral medication will not affect eligibility.

People wishing to access support under the Gestational Diabetes Supplies Benefit must meet the following eligibility requirements:
• Have diabetes mellitus
• Are a full-time resident of Alberta
• Have valid Alberta Health Care insurance registration
• Manage their diabetes through insulin therapy
• Have supplies and/or medication prescribed by a physician
• Do not have insurance coverage for any diabetes supplies, i.e. blood glucose test strips, injection supplies, and lancets (having insurance coverage for insulin or oral medication will not affect eligibility)
• Have received training in self-monitoring of blood glucose
