The following information is for the researchers currently receiving funding from the Canadian Diabetes Association.
Each form contains instructions on how it is to be submitted.
Personnel Awards Holders
Doctoral Student Research Award & Post-doctoral Fellowship Award
Annual Progress Report
Annual progress reports must be submitted to the Manager, Research at CDA by March 1 of each year, except the last year in which a Final report will be due within six months of the end date of the award. Second and third years of funding are contingent upon receiving this annual progress report.
CDA DSRA and PDF Progress Report
Final Report
A final report must be submitted within six months of completion of the award to the Manager, Research. This report should include the original title, names of awardee and supervisor(s), hypotheses tested, results and outcomes achieved, and relevance to diabetes. Also include a list of all presentations (oral or written), abstracts and/or publications in-progress or submitted, and copies of all abstracts and/or publications in press, that have resulted from this funding.

Clinician-Scientist Award
Annual Progress Report (Except Third Year)
CDA is tracking the impact Association funded personnel are making on diabetes research in Canada. As a result we have instituted an Annual progress report that must be submitted electronically by the Clinician Scientist to CDA no later than August 31 each year. Failure to submit an annual progress report will jeopardize future funding from CDA.

Third Year Progress Report
Fourth and fifth year funding for a Clinician Scientist is contingent on CDA receiving a detailed written progress report by March 1 in the third year of funding. The progress report will undergo review in the annual competition by the Personnel Awards Committee to assess progress. Upon recommendation from the Personnel Awards Committee, the Canadian Diabetes Association reserves the right to withdraw the final two years of funding if there is unsatisfactory progress reported.

Annual Progress Report
The Canadian Diabetes Association (CDA), as part of our Research Strategic plan, is tracking the impact that Association-funded research is making on those living with diabetes in Canada. In order to do this, we implemented a requirement in 2007 for all operating grant recipients to submit annual progress reports. Annual progress reports must be submitted to the Manager, Research at CDA within two months of the year-end of the grant (if there are no extensions on the grant, the deadline is August 30).
