Eli Lilly Graduate Scholarship

This scholarship is in recognition of outstanding individual effort in diabetes care and enables a DES member to pursue graduate or post-graduate studies in a diabetes-related field.

Purpose

The Eli Lilly Graduate Scholarship enables a Diabetes Educator Section (DES) member to pursue graduate or post-graduate studies in a diabetes related field.

The Award

Recipients of the scholarship will receive $5,000 paid in two installments: the first upon receipt of proof of acceptance in a program; the final upon completion of the program.

Eligibility

The scholarship is open to:

  • Must be a DES member in good standing for a minimum of 2 years and for the duration of the Scholarship
  • Must be a Canadian citizen or landed immigrant
  • Members of the DES Awards Selection and the DES National Executive are ineligible
  • An applicant must not have received another DES or CDA scholarship/bursary for the same academic year
  • Members of the DES Awards Committee and National Executive are not eligible for nomination

Requirements

  • Three letters of reference, including one from supervisor
  • An active DES member for a minimum of 2 years and for the duration of the scholarship
  • Must be a Canadian citizen or a landed immigrant
  • An applicant must not have received another DES or CDA scholarship/bursary for the same academic year
  • This award will not be awarded to the same applicant in consecutive years
  • Studies at Canadian universities will receive preference
  • A copy of the nominee’s curriculum vitae
  • DES National Executive and Awards Committee members are not eligible

    All completed applications and supporting material will be forwarded to the DES Awards Selection Committee. Applicants with incomplete applications will be informed that there submission will not move forward. The Selection Committee will select the successful applicants and all applicants will be notified of the outcome of their application by August 15, 2012.

    The scholarship award will be provided directly to the scholar by the award sponsor upon receipt of proof of acceptance in the program indicated on the application. The scholarships will be in effect for five years only. Funds will be issued in two installments (50% at the start of the program and 50% upon completion of the program if completed within the five year term). When you have completed the program, a copy of your report should be submitted to the sponsor and to the Canadian Diabetes Association to be submitted to the Canadian Journal of Diabetes and/or CDA’s Diabetes Communicator. In the event the scholar does not enroll as planned, the scholarship will be withdrawn.
