Roche Diagnostics Public Service and Education Award

In recognition of outstanding efforts by a DES member in promoting awareness of diabetes and developing programs to help people living with diabetes.

Purpose

The Roche Diagnostics Public Service and Education Award recognizes the efforts of diabetes educators who have done exceptional work in promoting awareness of diabetes and developing programs to assist people who must live with this condition.

The award recognizes a diabetes educator who excels in any of the following ways:

  • Volunteers to provide service to individuals with diabetes
  • Contributes to the development of programs for people with diabetes in the community (i.e. support groups for seniors or youths)
  • Advocates to government and industry for funding of programs related to diabetes
  • Promotes awareness and support of diabetes-related issues to public, government and industry
  • Initiates innovative ventures in the community to enhance the lives of people with diabetes

Nomination Process

Requirements

Applications must be typed or submitted in electronic format (i.e. Word document or .pdf) on official forms available through the DES. Handwritten, incomplete or late applications will not be considered.
In order to nominate a candidate, you must be a DES or C&SS member in good standing; the candidate of the award must be a DES member in good standing.
Include with the completed form:

  • two letters of recommendation
  • one current copy of the nominee’s resume or curriculum vitae

The Award

Recipients of the award will receive $2,000 and a commemorative plaque, presented at the the Canadian Diabetes Association Professional Conference and Annual Meetings held each year in October.

Eligibility

The award is open to:

  • An active DES member in good standing for a minimum of one year, who are Canadian citizens or landed immigrants.
  • NOTE: Nominators are not excluded from eligibility.
  • NOTE: Members of the DES Awards Selection Committee and National Executive are not eligible for nomination.

Letters of Recommendation

Letters in support of the nominee should be written by healthcare professionals, describing professional contact and attesting to the unique strengths of the nominee. They must be dated, signed and include the writer’s
name, title, address and telephone number. A .pdf of the letter containing an electronic signature is also acceptable.

As specifically and concisely as possible, the letters should include details of:

  • How do you know the nominee and for how long?
  • The individual’s involvement, interest and commitment to promoting awareness of diabetes and to developing programs designed to assist people who live with the condition.
  • Personal characteristics that make this individual uniquely qualified for this distinction.
  • A specific instance that highlights why you think the nominee should receive this award.

Notification

The recipient will be notified in sufficient time so they can arrange to attend the presentation ceremony at the annual Canadian Diabetes Association Professional Conference.
