Practical Diabetes Resource Award
In recognition of culturally sensitive educational tools that enhance the commitment to better diabetes care and education.

Purpose
The CDA/DES Diabetes Resource Award recognizes individuals who have developed a culturally sensitive educational tool for use in their practice.
The Award
Recipients of the CDA/DES Practical Diabetes Resource Award will receive $750 and a commemorative plaque presented to them at the Canadian Diabetes Association Professional Conference and Annual Meeting held in
October of each year.
Eligibility
The CDA/DES Diabetes Educational Resource Award is open to:
- An active DES member in good standing for a minimum of one year.
- The applicant’s name should not appear on the submitted educational material itself. Name of the applicant and institution must appear on the application form only.
- NOTE: Members of the DES Awards Selection Committee and National Executive are not eligible to apply.
Rules
- The culturally sensitive educational tool must be original in design and must have been developed by the individual submitting the entry. It must not be material developed by a commercial firm or a national organization. It may not be a tool that has already won a DES award or a revised version of such a tool.
- The educational material must contain accurate information and include a list of references, if appropriate.
- Applicants must provide a description of the tool and its purpose, which includes how it is used within the diabetes educator’s practice. The description should discuss the teaching/learning objectives of the tool
as well as the audience for which the material was developed; including the age range, ethnic group, literacy level, socioeconomic level, etc. - How you know the nominee and for how long.
- Describe the individual’s involvement, interest and commitment to diabetes education.
- Discuss what personal characteristics make this individual uniquely qualified for this distinction.
- Mention a specific instance that highlights why you think the applicant should receive this award.
- two letters of reference from people involved in diabetes education
- one current copy of the applicant’s resume or curriculum vitae
Letters of Recommendation
Letters in support of the applicant should be written by people involved in diabetes education. They must be dated, signed and include the writer’s name, title, address and telephone number. A .pdf of the letter containing an electronic signature is also acceptable.

Notification
The recipient will be notified in sufficient time for them to prepare their culturally sensitive educational tool for display at the Abbott Diabetes Care Booth at the Canadian Diabetes Association Professional Conference.
