DES Length of Service Award

Purpose

In recognition of CDA and DES volunteers by five-year increments.

Your submission must be postmarked no later than May 15, 2012.

The Award

Recipients of the DES Length of Service Award will be presented with pins and certificates for service in 5 Year increments at the Canadian Diabetes Association Professional Conference and Annual General Meetings held in October each year.

Eligibility

Nominees for this award must:

  • be an active DES member in good standing who is a Canadian citizen or landed immigrant
  • have volunteered professional service related to diabetes care at the national or local level (i.e. for Chapter Executive, National Executive, committee work, CDA Branch, CDA Area and Regional Leadership Centres)
  • have contributed work on her own time, not part of a job description/expectation
  • have contributed a minimum of 10 hours service in each year. Years need not be consecutive
  • NOTE: Nominators are not excluded from eligibility

Requirements

Nominations must be typed or submitted in electronic format (i.e. Word document or .pdf) on official forms available through the DES. Handwritten, incomplete or late applications will not be considered.
