Honorary Lifetime Achievement Award

In recognition of a long standing DES member’s contributions to improved quality of life for people affected by diabetes.

Purpose

The Diabetes Educator Section grants honorary lifetime memberships to recognize DES members whose long standing excellence in practice and significant volunteer contributions have improved the quality of life for people affected by diabetes.

Eligibility

Nominees for this award must:

  • be an active DES member in good standing.
  • have been an active DES member for a minimum of 20 years.
  • have made outstanding contributions to improving the quality of life for people affected by diabetes through providing excellence in diabetes education, management and care, advocacy and volunteerism (DES or CDA activities); contributions may have been at the national, provincial, territorial or local level.
  • volunteer professional service as related to diabetes care.
  • have played a leadership role in DES activities, or other activities related to diabetes education.
  • future plans of the indivucial to benefit DES and/or diabetes education.
  • NOTE: Nominators are not excluded from eligibility.
  • NOTE: Members of the DES Awards Selection Committee and National.
  • Executive are not eligible for nomination.

Requirements

Nominations must be typed or submitted in electronic format (i.e. Word document or .pdf) on official forms available through the DES. Handwritten, incomplete or late applications will not be considered.
The completed nomination form must be accompanied by 3 letters of recommendation.

Letters of Recommendation

Letters in support of the nominee should be dated, signed and include the writer’s name, title, address and telephone number. A .pdf of the letter containing an electronic signature is also acceptable.
As specifically and concisely as possible, the letters should include details of:

  • How you know the nominee and for how long.
  • The individual’s volunteer activities as they relate to diabetes care; national, regional or local chapter initiatives; leadership role in DES activities; and future plans of the individual to benefit DES and Diabetes Education.
  • Personal characteristics that make this individual uniquely qualified for this distinction.
  • A specific instance that highlights why you think the nominee should receive this award.

Submissions must be postmarked by May 15, 2012.

Notification

The recipient will be notified in sufficient time so they can arrange to attend the presentation ceremony at the annual Canadian Diabetes Association Professional Conference
