Lifescan Diabetes Educator of the Year
In recognition of a DES member who has made special contributions to diabetes education through dedication, sensitivity and innovation in the daily practice of patient education and care.

Purpose
The Lifescan Diabetes Educator of the Year award recognizes a health care professional who has made special contribution to diabetes education through dedication, sensitivity and innovation in the daily proactive of patient education and care. The award recipient has demonstrated a responsibility to:
- Continually strive to improve the quality of diabetes education
- Be current in their knowledge and skills regarding diabetes education and
- Respect the choices made by individuals affected by diabetes Standards for Diabetes Education in Canada 2009
- A DES member in good standing with a least 5 years of experience
- Note: members of the DES Awards Committee and the National Executive are not eligible for nominations
- How you know the nominee and for how long.
- A description of the individual’s involvement, interest and commitment to diabetes education.
- Personal characteristics that make this individual uniquely qualified for this distinction.
- A specific instance that highlights why you think the nominee should receive this award.
- Two letters of recommendation written by healthcare professionals describing the unique strengths of the nominee with examples
- Two letters indicating patient and community support
- A copy of the nominee’s curriculum vitae
- Note: DES members must be nominated by a DES member to be considered in the award selection process
The Award
Recipients of the award will receive $1,000.00, a commemorative plaque, and travel and accommodation to the Canadian Diabetes Association Professional Conference and Annual Meetings held in October each year.
Two guest speaking engagements at selected DES chapters are also included.

Letters of Recommendation
Letters in support of the nominee should be dated, signed and include the writer’s name, title, address and telephone number. A .pdf of the letter containing an electronic signature is also acceptable.

Notification
The recipient will be notified in sufficient time so they can arrange to attend the presentation ceremony at the annual Canadian Diabetes Association Professional Conference.
