CDECB Award of Excellence & Dedication

In recognition of a current CDE who demonstrates excellence, commitment and dedication in the delivery of diabetes care.

Your submission must be postmarked no later than May 15, 2012.

Purpose

This award recognizes a current Certified Diabetes Educator who demonstrates excellence, commitment and dedication in the delivery of diabetes care as well as the promotion of wellness and health in the community.

The Award

The recipient of the CDECB Award of Excellence and Dedication will receive a $500.00 cash award payable to the Award Recipient, one complimentary non-transferable re-certification fee (no cash surrender value) to be applied to the Recipient’s next CDE maintenance application and a commemorative plaque. The award will be presented at the CDA/DES Annual general meeting annually.

Eligibility

Nominees must:
• Be a member of DES in good standing for a minimum of five years.
• Hold a current CDE designation for a minimum of three years.
• Demonstrate volunteer experience in health care promotion (can be outside diabetes care).
• Currently working in diabetes care.
• Proof of registration with a regulatory body in Canada as a health professional.
Note: Members of CDECB Board of Directors, Award Committee and members of DES National Executive are not eligible.

Nominations Process

Requirements:
Applications must be typed or submitted in electronic format (i.e.: word document or pdf.) on official forms available through DES.
Letters of Recommendation:
Applications must include one letter of recommendation from a diabetes educators attesting to the specific attributes of the nominee.
Applications must include one letter of recommendation from a community-based health and wellness organization attesting to the volunteer role of the nominee.
Deadline for the award is May 1st of each year.

Approval Process

The decision to grant the award annually is at the discretion of the CDECB Awards Committee. It is to be noted one member of the DES Awards Committee (Director of Professional Development) is a member of the CDECB Awards Committee. The committee reserves the right to not grant the award in any given year if a suitable candidate is not identified. All decisions of the CDECB Awards Committee are final and not appealable.
