Diabetes Camp Educator Award

In recognition of a camp educators’ commitment and dedication to diabetes camping.

Purpose

The CDA/DES Diabetes CAMP Educator Award recognizes individuals who have shown commitment and dedication to diabetes camping.

The Award

Recipients of the CDA/DES Diabetes CAMP Educator Award will receive $750 and a commemorative plaque presented to them at the Canadian Diabetes Association Professional Conference and Annual Meetings in October each year.

Eligibility

The CDA/DES Diabetes CAMP Educator Award is open to:

  • An active DES member in good standing for a minimum of one year, who are Canadian citizens or landed immigrants.
  • All educators in summer camps, teen weekend camps and adult weekend camps.
  • NOTE: Members of the DES Awards Selection Committee and National Executive are not eligible to apply.

Letters of recommendation

Letters in support of the applicant should be written by people involved in diabetes camping. They must be dated, signed and include the writer’s name, title, address and telephone number. A .pdf of the letter containing an electronic signature is also acceptable.

As specifically and concisely as possible, the letters should answer the following:

  • How do you know the applicant and for how long?
  • Describe the individual’s involvement, interest and commitment to diabetes camping.
  • Describe the applicant’s current and previous diabetes education and work experience.
  • Describe the camp with which the applicant is associated.
  • Discuss what personal characteristics make this individual uniquely qualified for this distinction.
  • Mention a specific instance that highlights why you think the applicant should receive this award.

Requirements

Applications must be typed or submitted in electronic format (i.e. Word document or .pdf) on official forms available through the DES. Handwritten, incomplete or late applications will not be considered.

Include with the completed form:

  • two letters of recommendation from people involved with diabetes camping
  • one current copy of the applicant’s resume or curriculum vitae

Notification

The recipient will be notified in sufficient time so they can arrange to attend the presentation
ceremony at the annual Canadian Diabetes Association Professional Conference.
