Are you a person living with diabetes who is on insulin?

If so, we have two exciting opportunities for you!

The Canadian Diabetes Association (CDA) and St. Michael’s Hospital (SMH) are creating short videos to help educate and answer common questions for people living with diabetes who are starting insulin. To do this, we need help from people living with diabetes who are currently using insulin.


Interested individuals can help by:

  1. Auditioning to star in the video (see “Casting” below for more information)
  2. Participating in a focus group on the video content (see “Focus group” below for more information)
Everyone who auditions and/or is part of a focus group will be given an honorarium for their time.

Casting

About the video

  • The videos will feature real people living with diabetes as well as real health-care providers.
  • This project is in partnership between the CDA and SMH. The videos will be put up on the CDA’s YouTube channel and be used internally by SMH.

Who are we looking for

  • Men and women, 18 years of age or older, of all ethnic backgrounds, who have diabetes and are currently injecting insulin.
  • You must be able to arrange your own transportation to/from downtown Toronto. Tokens will be available, if needed.

Details and submission information

You will be asked to demonstrate parts of your insulin injection process on film, which could include loading the insulin pen and/or injecting insulin.

We will be filming the video at St. Michael’s Hospital (Li Ka Shing building, 209 Victoria St.) in downtown Toronto. Transportation will not be provided so you must be able to get yourself to set.

Filming date may be either Saturday, May 28 or Saturday, June 4, 2016(exact date to be confirmed). You will be required on set for two to five hours.

If you are interested in starring in our videos, please submit a video of yourself (see Video Instructions below).

Video submissions are due by: Sunday, May 15, 2016 by 9 p.m. ET (please submit ASAP as we will be reviewing submissions as we receive them)

Those chosen to star in this video will be paid an honorarium for their time. In addition, food will be provided on the filming day.

Video instructions

  • While looking in the direction of the camera, please clearly read out the audition script excerpt
  • Do not worry about memorizing the lines. It is fine to read the script off of a screen or piece of paper.
  • We do not expect the video to be professionally filmed. Filming yourself using a smartphone, tablet or webcam are all acceptable.
  • Please film in a quiet location with minimal noise distractions.

Submission instructions

  • Please e-mail your video to casting@diabetes.ca
  • Please write your last name and “CDA Insulin Video Submission” in the subject line (e.g. Smith - CDA Insulin Video Submission).
  • Please include your full name and phone number in the email. This information and your video will be kept confidential.
  • Please include answers to the following questions about your insulin regiment (this information will be kept confidential):
    • Do you use a reloadable or prefilled pen?
    • Do you use long or short needles? (Short = less than 8mm; Long = 8mm or longer)
    • When do you do your injections? (e.g. only at bedtime, breakfast and dinner, meals and other times)
If you are selected, someone from the CDA will call you by May 16, 2016.

Focus Group

Your personal experience taking insulin means you have many unique insights into what information people starting insulin need to know and what they may be feeling. Because of this, we need your help. We are looking for people on insulin who would be willing to be part of a focus group.

About the focus group

  • The focus group will involve watching parts of the video being acted out in front of the group. People will then be asked to share their opinions about what they saw with the group.
  • The group will run for one to two hours with six to ten people on insulin.
  • The focus group will be led by a CDA staff member.

Who we are looking for

  • Men and women, 18 years of age or older, of all ethnic backgrounds, who have diabetes and are currently injecting insulin.
  • You must be able to arrange your own transportation to/from downtown Toronto. Tokens will be available, if needed.

Details and submission information

We will be hosting multiple focus groups at the CDA’s national office in downtown Toronto (522 University Ave).Transportation will not be provided so you must be able to get yourself to the office.
Focus groups will be held on May 17, 18, 19 and/or May 2 (exact dates and times to be confirmed).

Submission instructions

  • Please fill out the below form by Thursday, May 12, 9 p.m. ET if you are interested in participating in a focus group.
  • If you are selected, someone from the CDA will call you by May 13.
It is important to us that the people who participate in the focus group represent the many faces of people living with diabetes. For this reason, we ask that you provide some basic information about yourself to enable us to ensure diversity within the groups. All information will be kept confidential and not shared with anyone outside of the CDA.

Those chosen will be paid an honorarium for their time.
